Rwanda Medical Supply Successfully Executes First Reverse Auction.

Rwanda Medical Supply has marked a significant milestone with the successful execution of its first reverse auction, aimed at addressing the critical shortage of blood bags in the country. This innovative procurement strategy not only resolved the immediate supply crisis but also resulted in substantial cost savings, ensuring a steady and affordable supply of blood bags for the future.
 
Background of the Initiative

This initiative arose from challenges faced with the existing contractor for blood bags, who requested price increases and prepayment, straining the procurement process. These conditions threatened to disrupt the supply of this essential medical commodity and as a swift and effective solution to secure the necessary supplies, RMS's senior management proposed a reverse auction.
A reverse auction is a competitive bidding process where sellers compete to offer the lowest price for goods or services to a buyer. Unlike traditional auctions, where buyers bid against each other in a reverse auction, the roles are reversed, and the sellers bid against other. This method, typically conducted online, is designed to drive prices down while ensuring transparency and fairness throughout the process.
Reverse auctions are particularly effective in healthcare procurement, especially for standardized, high-volume items like blood bags. When quality standards are pre-established, a reverse auction allows suppliers to focus on offering the best price. This approach helps institutions like ours secure essential supplies at lower costs while maintaining the quality necessary for safe healthcare.
 
Execution of the Auction

To tackle the blood bag shortage, RMS invited 16 global prequalified manufacturers to participate in the reverse auction. An online clarification session was held to address any questions from interested bidders, ensuring that all participants understood the auction's process and requirements. The auction itself took place under the oversight of RMS's Internal Tender Committee (ITC), with the presence of procurement managers and senior advisors to ensure transparency and adherence to procurement regulations.
During the auction, bidders submitted their prices through protected Google live documents, allowing for real-time bidding. This dynamic environment fostered competition among sellers, ultimately driving prices down and securing a reliable supply of blood bags at a lower cost.

The results of RMS’s first reverse auction were transformative. The auction achieved a remarkable 42% reduction in blood bag costs compared to traditional procurement methods.
The suppliers’ response was equally encouraging. They praised the process as straightforward, transparent, and fair, with all participants having a clear understanding of the criteria and goals from the outset.
 
Challenges and mitigations strategies

Reverse auctions can lead to significant cost savings in procurement by encouraging supplier competition, but they also present challenges. Key limitations include limited participation from smaller vendors, who may be deterred by the competitive environment and technical requirements. There’s a risk of focusing too much on price over quality, which can result in unsustainable pricing and reduced innovation. Additionally, reverse auctions may create transactional relationships rather than fostering long-term partnerships, and managing the auction process and ensuring fair play and transparency can be complex and time-consuming, often overlooking important factors like delivery reliability and after-sales service.
 
To mitigate these risks, RMS as well as other organizations should integrate reverse auctions with other methods that balance cost and quality. Establishing clear evaluation criteria, precise procurement specifications, and maintaining ongoing supplier engagement are crucial for ensuring quality and nurturing supplier relationships. Providing technical support to suppliers and ensuring a fair auction process can help address ethical concerns. By recognizing these challenges and implementing effective strategies, organizations can maximize the benefits of reverse auctions while minimizing potential drawbacks.
